Why your shoulders look broad or bulky, how TrapTox softens them, and what to expect from your first session
You notice it most in photos. Your shoulders slope up towards your ears, your neck looks shorter than it should, and off-shoulder tops never sit the way they do on the hanger. Gym sessions and stretching have not changed the shape, because the problem is not fat. It is muscle. TrapTox treats that muscle directly, with a few small injections and no downtime.
In short: TrapTox is an injection of botulinum toxin, a muscle relaxant, into the trapezius muscles that run from your neck to your shoulders. As the muscle relaxes and thins, the shoulder line looks softer and lower, and the neck looks longer. The muscle starts to relax within about a week, the slimming builds over 2 to 4 weeks and peaks at around 2 to 3 months, and results usually last 6 to 9 months. Many patients also say their shoulders feel less tight.
Below, we explain why shoulders bulk up, how TrapTox works, what the research shows, and what our patients have seen.

Why do my shoulders look broad or bulky?
The trapezius is a large, diamond-shaped muscle across your upper back. Its top part rises from the base of your skull to the tip of each shoulder. When it is large or overworked, it pushes the shoulder line upwards and makes the neck look short and thick. These are the causes we see most often:
| Cause | Why it matters |
|---|---|
| Genetics | Some people simply have a larger, higher trapezius |
| Desk work and phone use | Hours of hunching keep the muscle switched on and tight |
| Carrying heavy bags | A heavy handbag or laptop bag on one shoulder works the muscle all day |
| Stress | Many of us hold tension by lifting our shoulders without noticing |
| Gym training | Shrugs, deadlifts and heavy carries build the upper trapezius |
Stretching and massage relax the muscle for a while, but they do not make it smaller. TrapTox does.
Is it muscle or fat?
This matters, because the right treatment is different. A quick way to tell: shrug your shoulders and feel the area between your neck and shoulder. Firm tissue that tightens when you shrug is muscle, and TrapTox is the right fit. Soft tissue that does not change, often lower down at the back of the neck, is usually fat, which responds better to fat-reducing injections such as PB Serum. Your doctor will check this at your assessment.
How does TrapTox work?
Botulinum toxin blocks the signal from a nerve to the muscle it controls, so the muscle relaxes. A muscle that works less gradually becomes thinner, much like an arm that has been in a cast. In the trapezius, that means:
- a lower, softer shoulder line
- a longer-looking neck
- a more balanced shape between neck, shoulders and face
- less of the tightness that comes from a muscle that never switches off

We treat only the upper part of the muscle, with small, precise doses, so you keep normal strength for daily life.
What does the research say?
TrapTox is well established in aesthetic practice, but the published studies are small, so here is the evidence in brief. In a 2021 study in the Journal of Orthopaedic Surgery and Research, 20 women with a large trapezius had ultrasound-guided injections. The muscle was measurably thinner from 4 weeks and stayed thinner at every check up to 44 weeks. 80% of the women were very satisfied with the result and another 15% were relatively satisfied. One woman had temporary arm weakness that settled within about a month.
This matches what we see in clinic: change you can feel first, change you can see over the following weeks, and results that last for months rather than weeks.
What is the treatment like?
Your first visit starts with a consultation and shoulder assessment. We look at your shoulder shape, check whether the bulk is muscle or fat, and ask about your work, training and health. If TrapTox suits you, treatment can usually be done the same day.
| Quick facts | |
|---|---|
| Treatment time | 40 to 50 minutes, including numbing |
| Comfort | Numbing cream first; most patients feel only small pinches |
| Downtime | None; you can go back to work the same day |
| When you feel a change | The muscle starts to relax within about a week |
| When you see a change | Slimming builds over 2 to 4 weeks and peaks at around 2 to 3 months |
| How long it lasts | Usually 6 to 9 months |
| Maintenance | A repeat treatment when the muscle starts to return |
What to expect after treatment:
- First few days: small marks at the injection points fade within hours. You can carry on with your day.
- Week 1: the muscle starts to relax, and many patients notice their shoulders feel looser.
- Weeks 2 to 4: the shoulder line starts to look lower and softer in the mirror and in photos.
- Months 2 to 3: the full effect, with the slimmest shoulder line.
- Months 6 to 9: the muscle gradually returns, and you can plan your next session.

Is TrapTox safe?
In experienced medical hands, TrapTox is a low-risk treatment. Side effects are usually mild and short-lived:
- tenderness, small bruises or redness at the injection points
- a mild headache or tired feeling for a day or two
- temporary weakness when lifting heavy objects or raising your arms for long periods
Treatment is not suitable during pregnancy or breastfeeding, with an infection at the injection area, or for people with certain nerve and muscle conditions such as myasthenia gravis. We go through your health and medications at your consultation before any treatment.
Is TrapTox right for you?
You may be a good candidate if you:
- have shoulders that slope up towards your ears or look bulky in photos
- feel your neck looks short or thick
- want a softer, more feminine shoulder line for an event or for everyday confidence
- carry tension in your shoulders from desk work, heavy bags or stress
- train at the gym and want to soften overdeveloped upper trapezius muscles
A note from Dr. Adriene Chee: “Most patients come to us because of how their shoulders look in photos, and they are surprised by how much lighter their shoulders feel too. We start with a careful assessment, because a natural result depends on treating the right muscle with the right dose.”
Frequently asked questions
What is TrapTox?
TrapTox is an injection of botulinum toxin into the upper trapezius muscles. It relaxes and slims the muscle, which lowers and softens the shoulder line and makes the neck look longer.
How long does TrapTox last?
Results usually last 6 to 9 months. The slimming builds over the first 2 to 3 months, then the muscle gradually returns.
Does TrapTox hurt?
We apply numbing cream first, and the needles are very fine. Most patients describe a few small pinches.
Will my shoulders become weak?
We treat only the upper part of the muscle, so daily activities stay normal. Some people notice temporary weakness when lifting heavy weights or holding their arms up for long periods.
Can I still go to the gym?
Yes. We ask you to avoid strenuous exercise for the first day, and to avoid massaging the area or lying down for a few hours after treatment. Heavy shrugs and upper back training may feel different while the treatment is working.
Will TrapTox help with shoulder tension?
Many patients tell us their shoulders feel less tight. But studies of botulinum toxin for muscle pain have been inconclusive, according to a Cochrane review, so we treat this as a welcome bonus rather than a promise.
How much does TrapTox cost?
It depends on your shoulder size and the dose you need, so we give a personalised quote after your assessment. WhatsApp us for a quote.
