TCA CROSS for Ice Pick Scars

Treatment Overview

The TCA CROSS technique uses high-concentration trichloroacetic acid (TCA) applied directly into the base of ice pick or narrow boxcar scars. This controlled chemical injury triggers collagen remodeling, gradually filling and softening the scar.

how it works

A small amount of concentrated TCA is placed directly into the scar using a fine applicator. This induces a controlled reaction that causes collagen fibers to rebuild and lift the depressed scar.

Who Can Benefit

Ice pick scars are deep and narrow, so lasers and microneedling often cannot reach their base. In TCA CROSS, the doctor places a tiny amount of strong acid into each scar, which triggers new collagen at the base, so the scar becomes shallower over the following weeks.

  • Ice pick scars
  • Narrow, deep boxcar scars
  • A small number of distinct deep scars
  • Patients combining treatments for mixed acne scars

Wide, rolling scars respond better to PicoWay laser, subcision, Matrix RF microneedling or Fotona laser, and most patients have a mix of scar types, so we often combine treatments. In darker skin there is a risk of temporary darkening, so sun protection after treatment is important.

We postpone treatment during pregnancy and breastfeeding, or with a fresh tan.

Frequently Asked Questions

How many sessions will I need?

Usually 2 to 4, spaced 4 weeks apart.

Does it hurt?

Most patients feel brief stinging for a few seconds on each scar.

When will I see results?

Each scar becomes shallower over the weeks after each session, and the improvement builds with each one.

Is there downtime?

Treated scars turn white, then form small scabs that fall off in 5 to 7 days. You can wear makeup from the next day.
