A softer jawline, early jowls, a double chin that was not there before? How HIFU treatment (超声刀) lifts and tightens without surgery, what results to expect, what affects the cost, and how to choose a safe clinic
Medically reviewed by Dr. Adriene Chee, Medical Director, Verve Clinic. Last updated: 3 November 2026.
You notice it in photos first. The jawline is not as sharp as it used to be, the cheeks sit a little lower, and there is a softness under the chin that no amount of skincare seems to fix. You are not ready for surgery, and you do not want to look “done”. You just want your face to look firmer again.
That is exactly where HIFU treatment fits. In this guide, Dr. Adriene Chee, a doctor with more than 10 years of experience, explains how HIFU works, what the research shows, how it compares with Ultherapy, Thermage and Density RF, and what to check before you book.
What is HIFU treatment (超声刀)?
In short: HIFU treatment (high-intensity focused ultrasound, 超声刀) is a non-surgical lift. It focuses ultrasound energy into the deep layers of the skin, including the SMAS layer that surgeons tighten in a facelift. The heat triggers new collagen, which gradually lifts and tightens the face, jawline, double chin and neck over 4 to 12 weeks, with little or no downtime.
This guide is about HIFU for the face, neck and skin. HIFU is also used in hospitals to treat uterine fibroids and prostate cancer, which is a different medical treatment.
HIFU stands for high-intensity focused ultrasound. In Chinese, it is often called 超声刀, and many people in Malaysia know it as “Korean HIFU”, because many HIFU devices used in Asia are made in Korea. Whatever the name, the principle is the same: ultrasound energy passes harmlessly through the skin surface and is focused into tiny points at a set depth below it.
How does HIFU treatment work?
HIFU heats tiny points under the skin to about 65 to 75°C, according to a 2025 review in Applied Sciences. These are called thermal coagulation points. The skin surface is not damaged, because the energy only becomes strong at the focal point.
Different cartridges focus the energy at different depths:
| Depth | Layer treated | What it does |
|---|---|---|
| 1.5 mm | Upper dermis | Improves fine lines and skin texture |
| 3.0 mm | Deep dermis | Tightens the skin and supports collagen |
| 4.5 mm | SMAS (the muscle and fibrous layer) | Lifts the deeper structure that holds the face up |

The SMAS is the same layer that surgeons lift and tighten during a facelift. Reaching it without a cut is what makes HIFU treatment different from facials and most skin tightening devices. Over the following weeks, the heated points heal by forming new collagen, which is why the lift appears gradually rather than overnight.
What can HIFU treat?
HIFU treatment works best for mild to moderate sagging, when the skin has started to soften but has not yet folded or hung heavily. At Verve Clinic, we use HIFU treatment for:
- the full face, for an overall lift and firmer contour
- the lower face only, when the main concern is the jawline and jowls
- a softening jawline and early jowls
- a double chin and loose skin under the chin
- the neck
- body areas with loose skin, after an assessment
We also offer HIFU treatment for the eye area, to lift hooded brows and tighten the skin around the eyes.
Does HIFU really work? What the research says
Yes. HIFU treatment has been studied in clinical trials for more than a decade. Its results are gradual and more modest than surgery, but they are real.
- A review in Clinical, Cosmetic and Investigational Dermatology reported that 86% of patients had a clinically significant brow lift 90 days after a single treatment, and 77.7% were rated as improved at 180 days in another study.
- A study of 20 Korean patients in the Annals of Dermatology found improvement in all seven facial areas treated at 3 and 6 months after a single session, with the best results along the jawline, cheeks and around the mouth. Side effects such as redness, swelling and bruising settled within two weeks.
- In Malaysia, a 2024 study in the Malaysian Journal of Medical Sciences reviewed 56 patients at a Johor Bahru clinic who had HIFU combined with radiofrequency. 96.4% were rated as improved, and no serious side effects were reported, although the study was small and looked back at past records.
- The 2025 review notes that results usually become noticeable after 2 to 3 months and peak at around six months.
The research includes Asian patients as well as Western ones, which matters when choosing a treatment in Malaysia.

Is HIFU worth it?
A note from Dr. Adriene Chee: “Yes, HIFU is worth it. It is one of the most cost-effective treatments for skin tightening and lifting, with visible results.”
HIFU treatment is worth considering if you want a firmer, lifted face without surgery, needles or time off work, and you are happy with a gradual, natural change. It is not the right choice for heavy, advanced sagging, where surgery gives a better result. We will tell you honestly at your consultation which group you fall into.
HIFU vs Ultherapy vs Thermage vs Density RF
HIFU and Ultherapy both use focused ultrasound to lift the deeper SMAS layer; Ultherapy adds an on-screen image of the skin layers. Thermage and Density RF use radiofrequency to tighten the skin more broadly. HIFU suits lifting the jawline, jowls and neck, while radiofrequency suits overall skin tightening, and many faces benefit from both.
This is how the four treatments patients ask about most compare:
| HIFU | Ultherapy | Thermage (热玛吉) | Density RF (无双电波) | |
|---|---|---|---|---|
| Energy | Focused ultrasound | Focused ultrasound with an on-screen image | Radiofrequency | Radiofrequency |
| Main layer | Dermis to SMAS | Dermis to SMAS | Dermis and deeper tissue | Dermis and deeper tissue |
| Best for | Lifting the jawline, jowls, double chin and neck | Lifting the brow, jawline and neck | Overall skin tightening | Tightening the full face, eyes and neck |
| Usual plan | 3 sessions, 1 to 2 months apart, then maintenance | Usually one session | Usually one session | Usually one session, then maintenance |
| Downtime | Minimal | Minimal | Minimal | Minimal |
HIFU and radiofrequency complement each other: HIFU lifts the deeper structure, while radiofrequency tightens the skin above it. That is why we often combine HIFU with Density RF, and why a 2024 Malaysian study of HIFU combined with radiofrequency reported good results. You can read more in our Density RF guide.
What happens during HIFU treatment at Verve Clinic?
A HIFU treatment session at Verve Clinic takes 60 to 80 minutes, including numbing. Your doctor assesses your face, plans the depths and shots for each area, and then treats you while you stay awake and comfortable. Most people go back to work the same day.
- Assessment. Your doctor assesses your skin laxity, facial structure and goals, and confirms whether HIFU is the right treatment for you.
- Your plan. We decide which areas to treat, the depths to use and how many shots each area needs.
- Numbing. Numbing cream is applied to keep you comfortable.
- Treatment. The HIFU handpiece is moved across each area in lines. You will feel warm, prickling pulses, strongest along the jawline. We adjust the energy to your comfort.
- After treatment. You can go back to work the same day. Mild redness usually settles within a few hours.
How many HIFU sessions do you need, and how long do results last?
We usually recommend 3 sessions of HIFU treatment, 1 to 2 months apart, followed by maintenance. How often you need maintenance depends on your skin condition. In our experience, results last about a year.
| HIFU quick facts | |
|---|---|
| Treatment time | 60 to 80 minutes, including numbing |
| Usual course | 3 sessions, 1 to 2 months apart |
| Comfort | Warm, prickling pulses; numbing cream is used |
| Downtime | Minimal; back to work the same day |
| Common side effects | Mild tenderness for a few days; some people have mild swelling |
| When you see results | Gradually over 4 to 12 weeks |
| How long results last | About a year, in our experience |
| Areas | Full face, lower face, jawline, jowls, double chin, neck and body |
Is HIFU safe? Side effects and the “hollow face” worry
HIFU treatment is safe when a trained doctor uses a genuine device with the right settings. Most side effects are mild and short-lived:
- mild redness for a few hours
- mild tenderness for a few days
- mild swelling in some people
- temporary numbness, which is very rare in our experience
A common worry is that HIFU “melts fat” and makes the face look hollow. HIFU does produce heat, and the 2025 review links fat loss to overheating from incorrect settings or repeated passes over the same area. This is why HIFU should never be a one-setting-fits-all treatment. At Verve Clinic, we adjust the power and the number of shots to each face, skin condition and area, so the lift does not come at the cost of healthy volume.
If your face has already lost volume, HIFU treatment alone may not be enough. In that case, we may combine it with a collagen biostimulator such as AestheFill, or a small amount of filler placed carefully, as we explain in our guide to the overfilled face.
How much does HIFU cost in Malaysia? What affects the price
The cost of HIFU treatment in Malaysia varies widely. It depends on the areas treated, the number of shots, the device, whether the cartridges are genuine and how many sessions you need. That is why we give a personalised quote after your assessment, and why the cheapest option is rarely the best value. The main factors are:
- The areas treated. Full face and neck need more shots than the jawline alone.
- The number of shots. More shots at the right depths usually give a better lift, but only when the settings are correct.
- Genuine cartridges. Each cartridge delivers a limited number of shots. Genuine, unrefilled cartridges deliver the energy the device is designed for.
- The device. A genuine device, registered for use in Malaysia, costs more to own and maintain.
- A doctor’s assessment. A doctor decides whether HIFU suits you, which depths to use and how to avoid side effects such as fat loss.
- The number of sessions. A planned course with follow-up costs more than a single quick session, but it is what gives a lasting result.
Be cautious of very cheap HIFU, unlimited shots or treatment without a proper assessment. You cannot be sure what device is used, how much energy each shot delivers or who is responsible if something goes wrong.
Is HIFU at a beauty salon safe? Doctor-led vs salon HIFU
HIFU treatment is safest when a trained doctor assesses your face, uses a genuine registered device and adjusts the settings for you. Since 1 June 2026, HIFU devices used for aesthetic treatment have been regulated as designated medical devices in Malaysia under the Medical Device (Designated Medical Device) Order 2026. When the order was announced, the government said these devices should be operated only by people with qualifications and skills recognised by the government.
Before you book HIFU treatment anywhere, ask:
- Who will assess my face and perform the treatment?
- Which device will be used, and is it registered for use in Malaysia?
- Are the cartridges genuine and new?
- How will the settings be adjusted for my face?
- What happens if I have a side effect?
Who is HIFU suitable for?
HIFU treatment suits adults with mild to moderate sagging who want a gradual, natural-looking lift without surgery. You may be a good candidate for HIFU treatment if you have:
- a softening jawline or early jowls
- loose skin under the chin or on the neck
- flattening cheeks or lowered brows
- a wish for a gradual, natural-looking lift without surgery or downtime
HIFU is not suitable during pregnancy, or if you have an active infection or open wound in the area. If you have heavy, advanced sagging, we will tell you honestly that surgery gives a better result.
HIFU, filler or thread lift: which do you need?
HIFU treatment, filler and thread lifts solve different problems. HIFU lifts and tightens over weeks, filler restores lost volume immediately, and a thread lift gives an instant lift of sagging tissue. Many patients need a combination, which your doctor will plan at your consultation.
| Your main concern | Usually best treated with |
|---|---|
| Early jowls, softer jawline, loose neck skin | HIFU, often with Density RF |
| Hollow cheeks, temples or under-eyes | AestheFill or filler |
| Moderate sagging that needs an instant lift | Aptos thread lift |
| Dull, thin or crepey skin | Skin boosters |
| Heavy, advanced sagging | Surgery |
What can HIFU be combined with?
HIFU treatment lifts the deeper layers, but a natural result often needs more than one kind of treatment. Depending on your face, we may combine HIFU with:
- Density RF, to tighten the skin across the full face, eyes and neck
- Aptos thread lift, for an instant lift of the jowls and cheeks
- AestheFill and other collagen biostimulators, to restore volume and collagen
- Skin boosters, to improve skin quality, hydration and glow
- Matrix RF microneedling, for skin texture, fine lines and scars
Frequently asked questions
What is HIFU treatment?
HIFU (high-intensity focused ultrasound, 超声刀) is a non-surgical treatment that focuses ultrasound energy into the deep layers of the skin, including the SMAS layer that surgeons tighten in a facelift. The heat triggers new collagen, which gradually lifts and tightens the face, jawline and neck.
Does HIFU hurt?
Most people feel warm, prickling pulses, strongest along the jawline. We apply numbing cream first and adjust the energy to your comfort.
How many HIFU sessions will I need?
We usually recommend 3 sessions, 1 to 2 months apart, followed by maintenance. How often you need maintenance depends on your skin condition.
When will I see results from HIFU?
Some tightening can be seen soon after treatment, but the main lift develops over 4 to 12 weeks as new collagen forms.
How long do HIFU results last?
In our experience, results last about a year. Your skin keeps ageing, so maintenance sessions help keep the effect.
Is there any downtime after HIFU?
Very little. Mild redness usually settles within a few hours, and most people return to work the same day. Some feel mild tenderness for a few days, and some have mild swelling.
Will HIFU make my face look hollow?
Not when it is planned properly. HIFU produces heat, and too much energy in the wrong area can reduce fat. That is why we adjust the power and the number of shots to your face, skin condition and the area treated.
What is Korean HIFU?
Korean HIFU usually means a HIFU device made in Korea. These devices work on the same principle as other HIFU systems: focused ultrasound heats set depths under the skin to lift and tighten. What matters most is a genuine, registered device and correct settings.
Is HIFU the same as Ultherapy?
Ultherapy is a brand of HIFU (also called micro-focused ultrasound) that shows an ultrasound image of the skin layers during treatment. Other HIFU devices use the same principle without the on-screen image.
Is HIFU better than Thermage?
They work differently. HIFU uses focused ultrasound to reach deeper lifting layers, while Thermage (热玛吉) uses radiofrequency to heat and tighten the skin more broadly. Many faces benefit from both kinds of energy, which is why combination plans are common.
Is HIFU or Density RF better?
HIFU is best for lifting deeper layers, such as the jawline and jowls. Density RF tightens the skin across the full face, eyes and neck. They work well together, and your doctor will advise which to start with.
Can HIFU treat a double chin?
Yes. HIFU can tighten the skin under the chin and along the neck. Results depend on how much loose skin and fat is present, so your doctor will assess this first.
Can I combine HIFU with fillers or skin boosters?
Yes. HIFU lifts and tightens, while fillers restore lost volume and skin boosters improve skin quality. Your doctor will plan the timing of each treatment.
Who should not have HIFU?
HIFU is not suitable during pregnancy, or if you have an active infection or open wound in the area. Heavy, advanced sagging is better treated with surgery.
Can men have HIFU?
Yes. HIFU suits men who want a firmer jawline and neck without surgery or visible downtime.
Is HIFU at a beauty salon safe?
Since 1 June 2026, HIFU devices are regulated as designated medical devices in Malaysia, and they should be used only by people with government-recognised qualifications. Always ask who is treating you and what device is used.
How much does HIFU cost in Malaysia?
It depends on the areas treated, the number of shots and sessions, and the device used. WhatsApp us for a personalised quote after your assessment.
